Our Story


Born out of decades of working in the ER, we knew that acute care was broken. Between the nearly 80% of patients that did not need to be in an ER or Urgent Care and the fact that we were unable to follow up with patients and their acute problems, we were left with no other choice but to create a better solution. From our humble roots in Troy NY, we have grown to caring for millions of patients across the globe.

Keith Algozzine

Keith D. Algozzine, PA – C

Co-Founder, Chief Executive Officer


Keith Algozzine is a board certified Physician Assistant and co-founder and CEO of United Concierge Medicine and Management (UCM). Before creating UCM he has been the Chief PA of Emergency Medicine for St. Mary's Hospital in Troy, NY as well as part of the national ER start up management team for Pegasus Emergency Medicine. He was also a co-founding member and Director of Patient Services for a nationwide telemedicine company. Keith has been recognized by the the Albany Business Review as a 40 under 40 award winner and was instrumental in UCM being awarded the 2017 CEG Lift Off technology award. Keith currently represents United Concierge Medicine on the American Telemedicine Association Accreditation Advisory Board and has been appointed as a member/advisor to the Clinical Practice Guidelines Committees dealing with telemedicine considerations in the practice areas for primary care and urgent care. He now leads a team of telemedicine experts who are changing the way healthcare is delivered at United Concierge Medicine.

Micheal Bibighaus

Michael R. Bibighaus, M.D.

Co-founder, President & Chief Medical Officer


Dr Michael Bibighaus is a board certified emergency physician who has been practicing in the Capital District for more than 10 years. He grew up in Clifton Park, NY. As the son of a nurse and chemical engineer he knew he always wanted to pursue a career in Medicine. He went on to study biomedical engineering and economics at The Johns Hopkins University during which time he was also a member of the soccer team. He had the honor of staying at the school to earn his medical degree. Dr Bibighaus completed his postgraduate training at the world-renowned Johns Hopkins Hospital where he was Chief Resident of Emergency Medicine. Following residency he moved back to Clifton Park and has worked in Albany and Troy. He has been the Assistant Director of Emergency Medicine for a local emergency department for 8 years. He has also been the Medical Director of EMS services at the Times Union Center and a founding member of a national telemedicine company. He has also had the tremendous privilege of participating in medical missions to Cameroon and Kenya.

  • Rick Hirsh

    Chief Strategic Officer

    Rick brings UCM executive management experience, primarily in knowledge driven and technology enabled services companies. As a former Chief Executive Officer, active investor, and advisor in the mid-market, Rick helps build and fund companies with significant growth trajectories. Previously, Rick won the regional competition for Ernst & Young’s prestigious Entrepreneur of the Year, and Rick’s companies have consistently been recognized for growth by Inc. 500, Deloitte Fast 50, and other national analysts. In 2013, Rick founded Reclinata Group, an organization dedicated to building value through investment and operational advisory services. He has been active on multiple Boards of Directors, as well as Advisory Boards, for both national and global enterprises. Rick graduated from Brown University and later earned his MBA at New York University’s Stern School of Business.

  • Mark Burroughs

    Chief Financial Officer

    As an executive level financial manager, Mark brings a wealth of experience to UCM in all aspects of financial management, including as Chief Financial Officer of public and PE backed companies. Mark is able to leverage his experience in finance, operations, mergers & acquisitions and compliance to help companies grow profitably and generate cash. Over the last 25 years, Mark has worked for four companies, three of which were successfully sold. Mark started his career at Deloitte, rising to Senior Manager before leaving to pursue a corporate career. Mark has worked in a variety of industries such as biotechnology, financial services, professional services and distribution. Mark is a Certified Public Accountant and a member of the American Institute of Certified Public Accountants. Mark graduated from Montclair State University.

  • Lauren Algozzine, RN

    Chief Operating Officer

    Lauren is a Registered Nurse who graduated from the Samaritan School of Nursing. Right after graduation, Lauren worked at St. Peter’s Hospital on one of the cardiac unit. She then left to pursue a career in Emergency Medicine at St. Mary's Hospital where she quickly became the charge nurse of the department. During her 7 years in the ER, Lauren was a member of the QI committee and ED leadership committee. She began her journey with UCM as the Director of Client Services where she managed hundreds of UCM accounts and tens of thousands of patients. Now as COO she leads a team of dedicated staff in providing a concierge level of services for all business partners and patients.

  • Laura M. Balogh

    Vice President Benefits & Partner Development

    For over 30 years, Laura primarily spent her career negotiating, implementing and managing employee benefits for NYS’ largest public sector union and the 3rd largest health plan in the nation. She expanded her oversight to include over 1,200 local government municipalities and private sector employers and doubled the size of her Department. During her tenure, Laure created and served as Fund Manager for a labor/management Taft-Hartley Welfare Fund. She brings a wealth of benefit knowledge and a passion for finding solutions to the health care delivery system. Laura is a licensed Life/Accident & Health Insurance Agent and Broker and has served on various Boards, NYS Task Forces and Coalitions, including the New York State Public Sector Coalition on Health Benefits and the Capital District Health Care Coalition. She also served as part of the faculty for the International Foundation of Employee Benefit Plans and currently participates with the National Association of Health Underwriters.

  • Ester Horowitz

    Vice President of Strategic Partnerships

    Ester Horowitz is entrepreneurial spirit with 30 years of experience working in emerging healthcare markets. Ms. Horowitz is a recognized subject matter expert, author, and educator with an undergraduate degree in social work and a masters degree in business administration. A lifelong New Yorker, Ms. Horowitz came to healthcare initially by creating a local nonprofit mental health support group and serving as its executive director for three years. It led to becoming involved in the evolution of new verticals in primary, urgent, specialty and hospital care, Medicaid & Medicare managed care, compliance, population health & chronic disease both personally and professionally for organizations large and small. Ms. Horowitz prefers to be the person opening the doors and building the bridges between and within companies resulting in better ways to support & serve the greater good. She has B2B and B2C experience in sales & marketing, plus management, strategic design and development of new lines of business and expanding or creating companies. Her role as Vice President of Strategic Partnerships not only allows her to focus on building UCM's current business lines on local and national levels but also provide strategic leadership and exploration of UCM growth.
